I'm confused. There are four "first four" games. Two of them are between the "last four in" and two of them are between 16-seeds. UK could play the winner of one of the games deciding 16-seeds.

No, not usually although your logic is sound. The new first round is for autobid winners who are only there for an autobid conf. championship game. If duke was a last 4 in, they would be a 2 seed playing in Greenborough.

Typically the last 4 in are seeded in the 12-11 seeds. 10 at best but unlikely. UK won't play any last four in in 1st or 2nd round of NCAA tourney.

The last four in is just an ESPN thing. It has no bearing on seeding.

The first four is to determine two 16 seeds and two 12 seeds, so no, UK won't be playing Texas or Indiana.
