They increased the age ratio from 3:1 to 5:1. That ratio determines the max difference based on age you can charge for the same coverage. For example, if I charge a 20-year old $2k for plan x, I can't charge a 64-year old more than $10k for plan x. They offset that some by increasing tax credits as people get older. I listened to a radio story that stated that Kentucky got $2.5B with the expansion of Medicaid...hospitals that were on the verge of bankruptcy were reinvigorated, etc. That manner of payment will likely go away with this new plan. I know that governors don't get a vote but it will be an issue.

It's isn't 100% funded by the fed. It's between 50 & 70% depending on the need in the state. That means 70% in WV & KY. Anyone who enrolls between now and 1 Jan 2020 is grandfathered in. What changes in 2020 is that you go from a percentage of cost to a flat amount per person. States can still opt into the ACA funding until 2020 also. That's assuming that they don't opt to extend the current program past 2020 since that's an election year.Medicaid expansion was great for states in the short term, with the federal government picking up 100% of the cost, but as we approach 2020 that starts to decline, with states responsible for picking up the difference. And in a state like this, where we have a balanced budget amendment and are facing a significant deficit, that extra cost may be insurmountable.

Agreed. I think that part of this proposal is window dressing. They won't cut a program like that with a presidential election on the horizon.The problem is, once you start offering an "entitlement" you're never going to be able to take it away.
