Have a friend who went to high school with a girl who's name was Anita Dick.

I had a teacher named (by marriage) Mrs. Dick. I did not address her by that a single time, for the year I was in her class. Always went with the Ma'am approach.
I went to school with a girl with that name. I remember her name being called over the intercom. XD

Served in the USAF with a dude named John Hauf. Of course we called him Jack. He did not care for that name, not even a little bit.
Well technically it's Richard Pound. Probably had guys call him Dick as a joke to go with the last name and it stuck. For whatever reason they shortened names in the past and picked something that rhymed with it. For example Richard > Ric(k) > Dick or William > Will > Bill.
My mom was a NICU nurse, she had a patient one time named Anita Peace (not sure how it was actually spelled, just how it was pronounced).
So the lesson to be learned here is just don't name your kids Dick or Anita.
