Ophthalmology question for Doc

What type of eye malady would cause someone looking at 20,000 people in a crowd actually see 38,016 people? It can also cause 17,000 people to look like 50,000. I have taken into account that the 20,000 were from the fattest city in the nation, and take up multiple seats, but that doesn't explain the second example, that occurred a little north of here.
