I remember Dale Brown losing one like that once upon a time. I think it was John "Hot Plate" Williams who made the pass. The thing was they had the lead and he just thought the clock would run out when he threw it.

UCLA almost did something equally as bad
but luckily they didn't lose...
59 seconds left (or something close to that).... and they've got a full shot clock up by 3. They run the shot-clock down and don't even get up a shot the offensive possession was so bad. Still won though.
