So my question is... How should it be structured? What are your thoughts?
I'd structure it this way... Each coach gets 2 challenges a game. No play is reviewed without a coaches challenge. If the coach's challenge results in an overturned call, the coach gets a third challenge. If he's wrong, he loses his challenge (or looses... Depending on how you did on the act).
The ONLY additional plays that can reviewed outside the existing reviewable plays are safe / out and catch / no catch. Balks, obstruction, interference, infield fly... Any judgement call cannot be reviewed no matter how obvious it is. And just like football, only indisputable evidence overturns the call on the field.
