I believe it is talking about all the bullcrap we constantly stimulate ourselves with. It uses the metaphor of fist effing as a show of how we as humans pervert everything to the point where it loses it's true beauty. Basically we consume so much **** we don't know what is really beautiful or meaningful anymore. Given Tool's audience is mostly men the metaphor of super vulgar porn is a particularly good choice for making his point IMO.

I always took that song to be an analogy about being so numb to stimulants in society that we have to go bigger and to the extreme to even feel anything anymore.

Exactly, we try to fill ourselves with BS, but it doesn't work because it's not real so we never really feel alive.
