It comes down to geography, and of course has for years. Look at a map of all Eastern Kentucky and a huge portion of it is Mountains, take that same amount of space of th map of Texas, or Iowa, California or Mississippi and you have towns and people in the same amount of area. Less people , less students playing a game for local schools. That's why it's a barren area for football, or for that matter artists, computer code writers or door to door salesman.
As far as saying Eastern Kentucky tough is a different kind of thought than other places, ask yourself this if you have seen it yourself. Does that Texas roughneck, or Colorodo rodeo tough, or that south side Chicago head buster, or that NYC hustler, do they ride back under the mountain for 3 miles in a 29" seem of coal and work for 12 hours chipping chunks of block coal out half the day ? Until you've seen the genetic makeup year after year , time after time, rugged to the core then you really have not given credit where it's due. Many here can't even wrap their minds around just how God dang rough these people are and the balls it takes daily to do the jobs that are a way of life passed down through generations, but it helps to know so we can put it into perspective sometimes.
Yes there are healthier people and less obesity, and all the other bad things that are in these hills . But never think their are not some crazy rugged people from these hills. Still every now and then the world gets a glimpse of a Tim Couch and it's the rarity that makes them loved and respected that much more. It's kind of like finding a diamond in that coal field. One thing for sure is weather it's Jason Ledger or Tim Couch or Joey Coach or Jerremy Caudell that man is gonna give his all for the big blue. Maybe more so than a run of the mill recruit.
