The biggest choke I personally remember was three putting to shoot 70 in a qualifier for my club championship the summer between high school and college. I was in the 0-1 handicap range at the time but I've never broken 70.

I remembered that for a while because after going to college I stopped playing as much and for a long time I figured I'd blown every chance I had to break 70. I had shot 70 even 3 times. Recently (since ~2020) I've been playing more again and gotten back down to a 3, so I might have a shot to do it eventually. I've put together another 70 and a couple of 71s so hopefully I can manage it eventually.

I was up 4-0 on a kid from Millburn in a fencing meet and conceded 5 straight touches to lose the bout and cost us the meet. I still remember his name: Steve Mautone.

Made the match play Final 16 for club championship. Was seeded #16 and faced #1. Was up 4 holes with 5 to play. All I needed was to tie 2 holes. Had a birdie putt on 14 to end the match. 3 putted to lose the hole and went on to lose the next 4 holes.
Finals of a state tennis tournament doubles, playing with a new partner. Unseeded against a top seed. Won first set 6-0. Then led second set 5-0. I kind of had a crush on a female friend of his I met at the tournament who arrived at that point. Down 5-6 I asked her to leave. We won 7-6 for the championship. Never saw her again...
